What is Hypoxemia

Hypoxemia is a serious condition that requires immediate medical attention. Without sufficient oxygen (even if only within a few minutes), this condition can progress to hypoxia and damage to organs in the body, such as the heart, brain, kidneys, and other important organs that can be damaged and not function properly. Come on, find out more about hypoxemia, its causes, symptoms, and how to treat it.

Signs and Symptoms of Hypoxemia

Symptoms of hypoxemia can differ from one person to another, depending on the severity of the condition. Even so, the most common symptoms are:

  • Shortness of breath or heavy breathing.
  • Coughs.
  • Headache.
  • Dazed.
  • The heart beats fast.
  • Skin, nails, and lips turn blue (cyanosis).
  • Loss of consciousness or coma.

To find out whether the symptoms that appear refer to hypoxemia, an examination by a doctor is needed. In determining the diagnosis and finding the cause, the doctor will perform a complete physical examination, as well as supports, such as blood tests and chest X-rays.

Handling Steps to Overcome Hypoxemia

Treatment of hypoxemia aims to increase oxygen levels in the blood. The treatment steps taken will also depend on how severe the hypoxemia is experienced and the underlying cause. Some treatments that can be done are:

Oxygen therapy

Oxygen therapy using a mask or oxygenated tube (nasal cannula). If the patient is unable to breathe on his own, an intubation may be required to insert a breathing apparatus in the form of a tube into the patient's airway. After the breathing apparatus tube is installed, the doctor can pump oxygen through a special oxygen-infused bag (ambu bag), or use a ventilator machine.

 Administration of drugs

The choice of drugs can vary widely, depending on the factors causing the patient's hypoxemia. If it is caused by asthma or a narrowing of the airway, the doctor can give bronchodilators and corticosteroid drugs. For hypoxemia caused by infection, for example due to sepsis or pneumonia, the doctor can give antibiotics.

For hypoxemia caused by anemia or heavy bleeding, the doctor can provide treatment in the form of a blood transfusion. Because it is a severe condition that requires close medical monitoring and proper treatment, hypoxaemic patients will generally require treatment in the ICU.

What is the difference between hypoxia and hypoxemia?

Hypoxemia is low levels of oxygen in the blood, especially in the arteries. Hypoxemia is a sign of a problem in the circulatory or respiratory system that can cause shortness of breath. Meanwhile, hypoxia is low oxygen levels in body tissues as a result of low oxygen levels in the air. Hypoxia can have an adverse effect on body tissues, because lack of oxygen levels in the tissues will interfere with important biological processes in body tissues.

How do you tell the difference between the two?

Hypoxemia is determined by measuring the oxygen level in a blood sample taken from an artery or by measuring the oxygen saturation of your blood using a pulse oximeter. Normal arterial oxygen is 75 to 100 millimeters of mercury (mm Hg). Normal arterial oxygen levels below 60 mmHg usually indicate that your blood needs supplemental oxygen. While the reading with an oximeter can be said to be normal, it ranges from 95 to 100 percent. Oximetry values below 90 percent indicate that the oxygen level in your blood is low. Meanwhile, hypoxia is an advanced condition of hypoxemia, so if the oxygen level in the blood is low, it increases the risk of hypoxia.

Those are some explanations about hypoxekmia, signs and symptoms of hypoxemia and the difference with hypoxia. By knowing the disease, we can always be vigilant and maintain a healthy lifestyle.

What is Corona Virus

Corona virus is a virus that still related to the cause of SARS and MERS is exactly the crown. The shape of the crown is characterized by protein S in the form of shoes that are spread around the surface of the virus.

Corona viruses have one RNA chain so they are often called RNA viruses. This type of virus mutates faster than DNA by a million times.

Corona Paramyxovirus virus had appeared in the Google search engine. Both are different viruses, although both can infect humans from animals. Diseases caused by Paramyxovirus are Respiratory Syncytial Virus (RSV), Newcastle disease, and parainfluenza.

There are cases of Corona virus infection that show symptoms and some don't. For reported cases of Coronavirus the symptoms are:

a. Fever

b. Cough

c. Shortness of breath

According to the CDC, symptoms of the Corona virus may have been seen from 2-14 days. This estimate is based on the Corona virus incubation period in the MERS case. But unlike in the case of MERS, the 2019-nCoV infection can spread from patients who have no symptoms but have had close communication with other people.

Until now, research is still being done regarding the Corona 2019-nCoV virus and the best treatment for victims. known to date, Coronavirus is a large family of viruses that are found in some animals such as camels, cats and livestock. In some cases, the Corona virus infects humans and spreads as in the case of MERS, SARS and 2019-nCoV.

Quoted from The Guardian, victims who died of Coronavirus are generally old and have had health problems before. They have a weak immune system so they are easily infected with the Corona 2019-nCoV virus. But the Chinese government has five cases of death from the Corona virus that are less than 60 years old, namely 36, 50, 53, 55, and 58 years.

Therefore, it is very important to make preventive efforts to protect themselves and virus infections. Preventive efforts must be carried out from various ages, despite having good endurance.

Corona virus prevention efforts are increasingly important because until now the vaccine has not been found. Prevention of dealing with Corona virus must be done every day of all ages. The following efforts to prevent Corona virus are quoted from the CDC:

a. Wash hands with soap and water for at least 20 seconds, or a hand washing that contains at least 60 percent alcohol

b. Avoid touching your eyes, nose and mouth with unwashed hands

c. Avoid close contact with sick people

d. Stay at home if you are sick

Zone Diet

Zone diet is a way to reset the body's metabolic system by following the ancestral diet. The formula  is 40-30-30; namely 40% carbohydrate, 30% protein, and 30% fat. The problem of excess weight is always to be discussed by health experts. The proof is, curently so many types of diets which offer the weight loss in a short time. One of them is the zone diet.

The diet zone was first introduced by an American biochemist, Barry Sears, Ph.D ,. Barry argues that by diligently applying a zone diet, you can lose weight at least 1-3 kg a week. Barry stressed that the figure was fantastic because the weight that was reduced purely was fat weight, not water weight or muscle mass. Fat burning results from this diet will continue, even when you are asleep. This is because the main function of the zone diet is to improve the body's metabolic system. Since its introduction to the wider community, there have been many foreign artists who have revealed that this diet could be the dietary answer sought so far. Just name a few famous names, such as Jennifer Aniston, Madonna, and Demi Moore.

For sources of carbohydrates, this diet emphasizes the consumption of foods that are low in sugar and minimal process. What is meant by minimal process is food that does not go through the frying, baking, etc. So, if you really want to try this diet, then it's good to avoid sources of refined carbohydrates such as bread, pasta, and noodles. Instead, you are encouraged to increase consumption of fresh fruits and vegetables.

So what about protein? In the zone diet, the type of protein that can be consumed is a high-protein but low-fat food, such as skinless chicken breast, salmon, and some types of beans. Furthermore, to meet daily fat requirements, you are advised to consume food sources of unsaturated fats such as olive oil or almonds. Besides paying attention to the ratio and type of food consumed, you are also required to pay attention to meal times. Yes, during the zone diet program, you are encouraged to have breakfast at least 1 hour after waking up, eat with a span of 5 hours, and consume healthy snacks (such as salads or fruit pieces) before going to bed.

Diet zones are indeed worth trying. However, it should be noted that many individuals who fail to run this diet program because of the strict rules of eating ratio. In addition, the suggestion to avoid using excess salt is also often a barrier for those who want to try the zone diet. If you are unsure, it is better to consult your diet decision with your doctor.

E Coli, What is this?

US and Canada urged to avoid romaine lettuce amid E coli outbreak.  what kind of monster is he?

Escherichia coli or abbreviated E. coli is a bacterium commonly found in the human intestine. These bacteria consist of several types and most of them are harmless. That means that only a handful of types of E. coli bacteria can harm health.

One of the dangerous E. coli bacteria is E. coli O157: H7. These bacteria can cause food poisoning and infections that are quite serious. E. coli O157: H7 can produce poisons that can damage the walls of the small intestine and cause stomach cramps, diarrhea mixed with blood, and vomiting.

Factors that can increase risk
Here are some factors that can increase a person's risk of contracting an E. coli infection, including:
  • Age. Children, pregnant women, and elderly people are prone to suffer from diseases caused by coliosis and more serious complications from these bacteria.
  • Weakened immune system. People with AIDS or people who are undergoing chemotherapy will be more at risk for coli infection.
  • Decreased acid levels in the stomach. Acid produced by the stomach can provide protection against coli bacteria. Some gastric acid relievers such as esomeprazole, pantoprazole, lansoprazole, and omeprazole have the potential to increase the risk of E. coli infection.
Symptoms of E. coli infection
Symptoms of infection with E. coli bacteria usually begin to be felt three to four days after the body is exposed to these bacteria. But symptoms can also appear a day or even a week later. The following are symptoms that commonly occur due to E. coli infection:
  • Abdominal pain
  • Diarrhea, with mild to severe severity, and even bleeding.
  • Loss of appetite.
  • Nausea and vomiting.
  • Fever.
  • Fatigue.

In general, E. coli infections can be treated at home and can recover in a matter of days or one week. However, it is recommended to immediately see a doctor if someone experiences severe symptoms as follows:
  • Diarrhea that does not show signs of improvement after five days in adults, or for two days in infants and children.
  • Fever accompanied by diarrhea.
  • Vomiting for more than 12 hours. If it occurs in infants under the age of three months, immediately see a pediatrician.
  • The appearance of symptoms of dehydration, such as the amount of urine decreases, feels very thirsty, or consciousness decreases.
  • Cannot maintain fluid in the body.
  • Abdominal pain does not disappear after defecation.
  • Having intestinal infections after traveling abroad.
  • Stool mixed with pus or blood.

Diagnosis and Treatment of E. coli Infection
To diagnose an infection with E. coli bacteria and related poisons in the patient's body, the doctor will carry out laboratory tests through stool samples. Treatment will be recommended according to the conditions experienced.

For those who have mild infections, treatment can be done at home by consuming enough water, fruit juice, or clear soda drinks to prevent dehydration. Avoid apple juice, pears, coffee and alcoholic beverages during the recovery process.

In addition, consume soup or mild food when symptoms are still felt, followed by low-fiber foods such as eggs, rice, or bread when conditions start to improve. Avoid foods that contain milk, fat, high fiber, or spicy foods so that the symptoms don't get worse.

E. coli infections, both in the bladder (cystitis) and gastrointestinal tract, usually do not need antibiotics. Patients only need to consume large amounts of water to replace fluids lost due to diarrhea and vomiting, and take adequate rest.

To overcome dehydration in children who have diarrhea, ORS can help restore fluid in their bodies. In addition, ORS also functions to replace sodium, potassium, and also glucose lost from the body. Do not give antidiarrheal drugs that can slow down the digestive system because this drug will prevent wasting toxins out of the body.

If the E. coli infection that occurs is quite serious and causes uremic hemolytic syndrome, patients usually need to get intensive care at the hospital.

Choice of Stroke Treatment

Treatment for a stoke must begin as soon as possible. The faster the emergency treatment starts, the greater the chance of preventing permanent damage. Treatment depends on the type of stroke that attacks the patient, whether it is an ischemic stroke or a hemorrhagic stroke.
Emergency treatment for ischemic stroke
Emergency treatment for ischemic stroke must begin no later than 4.5 hours after the stroke. Ischemic stoke is the most common type of stroke, which is affected by blood clots in the brain. Treatment will be done to unravel the blockages that interfere with blood flow to the brain.
1. Aspirin
One of the most common treatments that doctors use in emergency stroke is aspirin. It has been proven effective to thin the blood, aspirin can help deliver blood to the affected area. However, you or other family members should tell your doctor if the patient is already on aspirin for heart disease or other diseases.
2. Stroke medicines
Your doctor may also give other drugs to break down the blockage. These drugs can be injected through a thin tube (catheter) into your veins. The most commonly used drug to treat stroke is tissue plasminogen activator (TPA). This drug helps break down blockages that occur in the brain. Other drugs taken and may be used to thin the blood and reduce the risk of future strokes are clopidogrel and warfarin. Statins have also been shown to reduce the incidence of stroke.
3. Catheter embolectomy
If drugs fail to break down blood clots, and if the stroke is centered on one area (acute), the doctor may use a catheter to reach the blockage and lift it manually using a special device. The catheter is passed through a blood vessel to the area where the blockage occurs. The blockage is then lifted using a device that resembles a wine bottle opener that is placed at the end of the catheter, or with a drug that breaks down through a catheter.
4. Decompression craniotomy
Severe stroke can cause serious swelling of the brain. Interventions through surgery may have to be done if drugs cannot reduce this swelling. The purpose of decompression craniotomy is to prevent the pressure inside the skull from enlarging to a dangerous level. In this procedure, the surgeon will open a small portion of the skull in the area of swelling. When the pressure is gone, this opening will be returned.

Prevention of ischemic stroke
After an emergency action, the doctor will evaluate the health of your blood vessels and determine what should be done to prevent further stroke.
Lifestyle changes
Post-stroke prevention generally focuses on improving heart health. This means lowering blood pressure or better managing cholesterol and fatty acids (lipids). You may need to combine exercise, a healthy diet, and medication.
Carotid endarterectomy
Carotid endarterectomy is usually done in patients who show symptoms similar to stroke such as transient ischemic stroke (TIA) or known as a mild stroke. In this procedure, the surgeon will remove plaque and blockage of blood from the vessels in your neck. This operation carries the same risk as other surgeries, but can also trigger a further stroke if the plaque or blockage of blood is released during the operation. Doctors usually take precautions to reduce this risk.

Treatment for hemorrhagic strokes
Unlike ischemic strokes, treatment for hemorrhagic strokes does not involve blood thinning drugs. Thinning blood will actually increase the amount of blood lost from the brain. If you are already on medication with blood thinners, your doctor may give other drugs to fight this effect or slow down blood pressure so that bleeding in the brain slows down.
1. Operation
Depending on damage to blood vessels in the brain, surgery may be needed after you have a bleeding stroke. Operations can not only repair damage, but also help prevent future problems. However, the area attacked by a stroke must be close enough to the surface of the brain so that the surgeon can access the blood vessels.
If the surgeon can access the affected blood vessel, he can remove it through surgery. This can reduce the risk of blood vessel rupture in the future. However, depending on the location of the aneurysm, removal through this operation may not be possible.
2. Coiling
If a damaged artery is not accessible through surgery, catheterization is your choice. Using a catheter, different experts will use a technique called coiling or aneurysm embolization. After the surgeon finds a broken vessel, he will release a roll of wire to the area. This wire is made of soft platinum, its size is smaller than a strand of hair. This wire will function as a net for blood clotting and closing holes from other arteries.
3. Trimming aneurysms
Your doctor may recommend trimming your aneurysm by permanently attaching clamps to prevent further bleeding or even rupture of blood vessels. Aneurysm pruning is a surgical procedure and is usually only recommended if the coiling is not expected to be effective. Pruning is a more invasive action than coiling.
4. Rehabilitation after a stroke
The healing process after a stroke depends on the extent of the damage and which part of your brain is affected. For example, if a stroke occurs in the right side of the brain, you might need physical rehabilitation that focuses on walking up and down stairs, dressing alone, or bribing food into your mouth, because the right side of the brain controls visual-spatial functions. You may also need to undergo rehabilitation or corrective action to help with breathing, vision, bowel and bladder control, talking, and other problems.
